Phobia the fear within

A phobia is a clinical term that is used to describe an irrational and persistent fear of certain objects, situations, activities, or persons. These fears are beyond one's control and may interfere with one's daily activities. Phobias belong to a large group of mental problems known as anxiety disorders, other conditions included in this group are obsessive compulsive disorder, panic disorder, and post traumatic stress disorder. There are many forms of phobias, and they can be a fear of something specific such as flying or social fears that may cause certain individuals to feel anxious in social situations. The quality of hospital food

In this article I write about the quality, or lack of it, in the food that is served in some of the UK hospitals. My nan has recently died in hospital which has been very upsetting for my family. She was hit by one of the famous hospital bugs and became very weak. She was in need of some decent food which would have helped to build up her strength. What she received was basically so poor and tasteless that she found it hard to eat.


Despite many attempts by various members of the family to pursuade her to eat more, she could not stomach it and kept saying that it the food had no taste and that it was disgusting. I have to admit that it was not exactly tempting when I saw what was being dished out.


In the UK we are very lucky in the standard of our hospitals when you compare it to many other countries in the world. Our country is very rich but a lot of this cash is being wasted on certain projects. Surely people in our hospitals deserve a much higher standard of food, food which they can look forward to eat.


I have recently watched a program about how Jamie Oliver is trying to improve the standard of food in schools throughout the UK. This is a very worthwhile project and as a father of two children myself, I am more than aware that the canteen food served in most schools is not up to scratch. At the schools where my own children attend, the parents have the option of paying for these meals or they can make their child a packed lunch. We choose the packed lunch option.


In hospitals there is no such option. I would have loved to have been able to cook some food for my nan, which I could then have taken to her when I visited. I would have also liked to have taken some cleaning products as the place could have done with some tender loving care as well as a lick of paint.


A message to Jamie Oliver, you are doing a sterling job but please could you turn your attention to the hospitals next!


A message to Tony Blair, stop wasting money killing our troops in Iraq and start looking after your own!


I am not trying to say that my nan would have survived if the standard of food had been better in the hospital where she spent her final days. What I am trying to say is that I hope it improves in a big way before I have to spend any time there.

6. Failing to get frequent policy reviews and updates. Your financial life isn’t static, and neither are your insurance needs. The value of a collection may increase; extensive home renovations could mean a sharp rise in the value of your property; and the re titling of assets as part of your estate plan—or because of divorce, a death in the family, or the birth of a child—could necessitate policy changes. Even lacking major events, you probably need a comprehensive review of all your insurance coverage at least every two years.

Pots and pans ammunition used against british in the battle of north point at baltimore

Commercial ports at various locations on the upper Chesapeake Bay were founded during the 17th century and called “Baltimore.” In 1706, the Maryland colonial General Assembly created the port at Locust Point as a tobacco port of entry. The present city of Baltimore dates from July 30, 1729 and is named after Lord Baltimore. Lord Baltimore was the first Proprietary governor of the Province of Maryland.


Baltimore’s growth seem to be boundless in the mid - to late 18th century as a granary for sugar-producing colonies in the Caribbean. This growth was partly due to the fact that Baltimore was shorter in distance from the Caribbean as compared to other large port cities such as New York City and Boston, which reduced transportation time and minimized the spoilage of flour. The profit gained from Baltimore’s sugar-producing abilities encouraged the cultivation of cane and the importation of food.


Baltimore and other cities joined together in protesting Boston’s punishment by the British and signed agreements to not import from or export to Britain due to the British taxation. Prominent city leaders such as Jonathan Plowman Jr. encouraged the city to join the resistance to British taxation. These agreements enabled Baltimore to play a key part in the events leading to and including the American Revolution.


The British declared Baltimore a “nest of Pirates” during the War of 1812. The British burned Washington, D. C., however, the American forces won by repulsing joint land and naval attacks.


The British landed at Sparrows Point and when the local residents run out of ammunition, they threw pots, pans and anything else they could get hold of at the British. At the Battle of North Point, local residents killed the British commander, General Ross. General Ross thought he would the battle and made the strong statement of: “We will win this battle or I eat dinner in hell tonight.” The British lost this battle. British reinforcements were not possible and their forces just withdrew. This naval engagement inspired Francis Scott Key to write the poem “The Star-Spangled Banner,” which became the lyrics to the United States’ national anthem. The battled was memorialized in the Battle Monument on the city seal.


Following the Battle of North Point, Baltimore’s population grew significantly. This growth was attributed mainly to the increased commerce abroad and more importantly with points west in the interior of the United States. Two major transportation accomplishments of the federally funded National Road and the privately funded Baltimore & Ohio Railroad made Baltimore a major shipping and manufacturing center. Fortunes were made and the city’s distinctive local culture started taking shape. Baltimore started to develop a unique skyline peppered with churches and monuments. John Quincy Adams visited the city in 1827 and nicknamed it “Monument City,” a nickname that remained popular for well over a century.


Baltimore was separated from Baltimore County in 1851 and became an independent city at that time.

What is e85 ethanol and how is it made

With gasoline prices rising higher and higher, with no relief in sight, consumers are becoming more aware of and more interested in alternative fuel sources. One of those viable fuel sources is E85 ethanol. These days, there is quite a bit of talk about this fuel, and why shouldn’t there be? After all, it costs close to thirty five percent less than gasoline and is quickly becoming a preferred choice for motorists at the pump stations. E85 ethanol is already widely used in Brazil with about 90 percent of Brazil’s vehicles able to use E85 ethanol. Although a much lower percentage of vehicles in the United State’s are flexible fuel vehicles, and therefore able to burn E85 ethanol, it is quickly gainer favor.


What is E 85 Ethanol?


When 15 percent of leaded gasoline is combined with eighty five percent of ethanol, the result is the low-cost E85 ethanol fuel. It is an alcohol-based fuel that can be produced from employing the use of two methods. The first source of production involves the fermenting and distilling of starch-like feedstock. They include corn, barley, and wheat. The next method, which is referred as bio-ethanol, involves extraction from trees and grasses.

Natural listings. The listings that search engines do not sell. Instead, sites appear solely because a search engine believes it is important for them to be included, regardless of payment. Note that paid inclusion listings are still treated as natural listings by many search engines.


Outbound links. Links on one website that lead to other websites.


Paid inclusion. An advertising program where pages are guaranteed to be spidered and included in a search engine's index in exchange for payment.


PPC. Pay-per-click – means the same as cost per click (CPC).


Paid listings. Listings that search engines sell to advertisers, usually through paid placement or paid inclusion programs.


Pay-for-performance. A term popularized by some search engines as a synonym for pay-per-click. It stresses to advertisers that they are only paying for ads that "perform" in terms of delivering traffic, as opposed to CPM-based ads, where ads cost money even if no-one clicks on them.


Paid placement. An advertising program where listings appear in response to particular search terms, with higher rankings typically obtained by paying more than other advertisers.


Rank. The order in which web pages are listed in search engine results.


Reciprocal link. A ‘link exchange’ in which two sites link to each other.


Results page. The page that appears after a user enters their search terms.


Robots. txt. A file used to keep web pages from being indexed by search engines.


Search engine. A service designed to allow users to search the web, or another database of information.


Search engine marketing (SEM). Marketing a website using search engines, whether you’re improving your ranking in natural listings, purchasing paid listings or some combination of the two.


Search engine optimization (SEO). Altering a website so that it ranks higher in the search engines.


Search terms. The words a searcher enters into a search engine's search box.


Shopping search. Shopping search engines allow shoppers to search the web for products and their prices.


Spam. Any search engine marketing method that a search engine decides is detrimental to its efforts to deliver relevant search results.


Spider. See crawler.


Submission. The act of sending a URL to a search engine, for inclusion in its index.


XML feeds. A process in which information about a page is fed to the index without using a crawler, for example using RSS.


The best advice is to follow a good search engine promotion system. Keep track of when you submit your sites and how soon they’re indexed -- checking once a week is sufficient.


Ranking systems can be confusing and there are often complex factors involved, but you do not need to be an expert in the field to achieve top results. Take a chance – after all, you have nothing to lose.

Skin care for adult women with acne

Acne affects some 25% of men and 50% of women at some point in their adult lives. The treatment of acne can be difficult for some women who also want to pay attention to moisturizing their skin.


Acne in women can be brought on by many different factors. Some are easy to figure out. The use of heavy cosmetics can bring on acne. This is simply remedied now. There are moisturizers and cleansers out now that take into account the needs of maturing skin along with being effective against acne. Make sure that you follow the skin care regimen suggested by the manufacturer of any product you use. Regular facials can help to reduce acne flair ups. Pheochromocytoma symptoms diagnosis and treatment

Pheochromocytoma is an important cause of secondary Hypertension. We have adnenal gland located at upper portion of each kidney. It is divided into adrenal cortex and adrenal medulla. Pheochromocytoma is a tumor of Adrenal Medulla. Normal function of adrenal medulla is to produce epinephrine or adrenaline. Which is responsible for controlling blood pressure and to help cope with stressful situations.
So majority of symptoms of pheochromocytoma are due to excess secretion of adrenaline from adrenal medulla. Most patients of pheochromocytoma have recurrent episodes of headache, sweating and a feeling of high anxiety. The following symptoms are listed from the most common to the least common
Headaches (severe)
sweating Episodes (generalized)
heart palpitations (tachycardia and palpitations)
Anxiety
nervousness (feelings of impending death)
tremors
Pain in the lower chest or upper abdomen
Nausea (with or without nausea)
Weight loss
Heat intolerance

These symptoms may come by situations which causes pressure over tumor like physical activity, exercise, defecation, anesthesia, or change in body position.

Who should be examined for Pheochromocytoma
Those who have uncontrolled hypertension
Those who have age between 40 and 60
Those who are already taking 4 antihypertensive drugs but blood pressure is not controllable.
Those who have episodes of above symptoms

Cause of pheochromocytoma is unknown, however some forms of this tumor runs in families like,
a-Multiple endocrine neoplasia, type II (MEN-II). In addition to a pheochromocytoma, people with MEN-II also have thyroid cancer. Other forms of MEN-II include pheochromocytoma with thyroid cancer and hyperparathyroidism (MEN-IIA), and pheochromocytoma with thyroid cancer and tumors of nerves in the eyes lips, mouth and digestive tract (MEN-IIB).
b-Neurofibromatosis 1 (NF1). Pheochromocytomas can occur in a small percentage of people with NF1, a syndrome that includes multiple tumors in the skin (neurofibromas), pigmented skin spots, tumors of the optic nerve of the eye, and bone diseases.
c-Von Hippel-Lindau (VHL) disease. People with this rare multi system disorder are at high risk of pheochromocytoma brain eye and kidney tumors.

Most pheochromocytomas are benign tumors and they don't spread to other parts of body however some forms do show metastasis (spread) to other parts of body like lungs, bones and brains. Usually only one gland is involved however this tumor can be present in both adrenal glands.

Investigations:
Blood and Urine tests: We perform blood and urine tests to diagnose pheochromocytoma. 24 hr urine collection is tested for epinephrine, norepinephrine and dopamine. This test is called VMA. Idea is just to check level of epinephrine and its metabolites in urine. If these are detected in urine in excess quantity, then tumor is diagnosed.

Abdominal scan.
Now you have diagnosed tumor by blood and urine tests, after that to find out location of tumor, do Ultrasound, CT scan of abdomen.

Complications:
If high blood pressure remained uncontrolled it may lead to complications of heart failure, infarction, cerebrovascular accident, vision damage and kidney failure.

Treatment:
First treatment is to control high blood pressure with medication like Alpha Blockers and Beta Blockers. When epinephrine is secreted by this tumor it acts on alpha and beta receptors present on heart and blood vessels, result is vasoconstriction and increase heart rate. Now if if we take drugs like alpha blocker and beta blocker, epinephrine will be blocked by these drugs to act on these receptors so result is vasodilation and slow heart rate. Common Alpha blockers are Prazosin (Minipress), Terazosin (Hytrin) Phenoxybenzamine( Dibenzaline). Common Beta Blockers are Atenolol(Tenormin), Carvedilol(Carveda), Metoprolol(Mepressor), Inderal.

Treatment of choice for this tumor is Surgery. After removal of this gland by surgery, blood pressure becomes normal with a day. There are two types of surgeries. General and Laproscopic surgery. However Surgery is not an option for those tumors which show metastasis to other parts of body. For that we use chemotherapy and radiations.

Afghanistan nato s classic self-defeating exercise

If you think that you are aware of what’s going on in Afghanistan today, think again. The very people—politicians, generals and journalists--feeding you information on the Afghan matrix are unsure about both, their facts and their conclusions.


A few days after it was announced that Afghanistan’s opium production may well exceed 53% of national GDP in 2007, an investigative reporter asked an officer inside the UN’s Office on Drugs and Crime about the impact (i. e. the futility) of hundreds of millions of dollars spent on poppy eradication and crop substitution schemes since the Taliban were forced out of Kabul in late 2001. “We don’t appear to achieve anything credible,” the officer acknowledged on condition of anonymity. “In fact, we just do our best, year after year.”


An Afghan volunteer supporting NATO’s drug eradication programmes was a bit more precise when confronted with the hard fact that more than 660 tons of opium and morphine, valued at US$4 billion, have been exported from Afghanistan last year. “We keep telling the small farmers to grow poppy substitutes and they keep telling us that they don’t own the land, they don’t have title,” the volunteer revealed in a private conversation. “It is obvious that the warlords and clerics determine who grows what and where, not the farmers we keep talking to daily.” The volunteer appeared to have no idea whether the various groups currently directing crop cultivation—drug kingpins, pro-Taliban Mullahs and religious radicals—were working in conjunction or otherwise.


In 1978, there were hardly any poppy fields in Afghanistan, despite the fact that the trade in drugs was flourishing in Pakistan’s North West Frontier Province. In 1978, despite a deep-rooted feudal structure in the countryside, where the majority of Afghans lived, Afghanistan was substantively self-sufficient in food. In 1978, the Afghan government determined that economic and social progress in Afghanistan could only take place if (a) land reform laws were effectively implemented, (b) the huge rural debt due to moneylenders was cancelled without delay and (c) adequate rural credit was made available to small farmers.


After all, as a foreign economist visiting Afghanistan at that time concluded that “without fundamental changes in the agrarian milieu, this country will not achieve any breakthrough whatsoever. This is primarily an agricultural country, and a complex one at that, and there is now an urgent need to sharply increase investments in rural infrastructure, in order to raise productivity, and to improve the quality and packaging of output.”


But that vision of an economically resurgent Afghanistan was in tatters well before the reform agenda threatened to become a viable reality. The anti-Soviet Jihad of the 1980s set Afghanistan on a different path altogether. Land records were destroyed in more than 80% of the provinces. Freshly-armed tribal chiefs forced sharecroppers and landless labourers to grow poppies instead of grains, fruit and vegetables. Militias under the umbrella of self-proclaimed freedom fighters, like Osama Bin Laden and Gulbuddin Hekmatyar, opened up new drug smuggling routes to Central Asia, the Middle East and Europe. And, in the midst of the complete transformation (i. e. a deterioration) in Afghan agriculture over a 5-6 year period, village Mullahs, many of whom landowners in their right, were telling poor Afghans that “only Allah can give you land, not the communists.”


Today, twenty years later, the issue of land titles is mired in even greater confusion. Some of the refugees returning home from Pakistan and Iran have indeed been allotted land; but, in the absence of access to reasonable credit, refugee families are rapidly falling into the same old cycle of perpetual debt. Land officials in Kabul, attempting to formalize a land title regime to be applied throughout Afghanistan at some point in the future, are confronted by the challenge of possession. “Here possession is 100% of the game, and we all know who has possession of the vast majority of fertile land,” a UN aid worker explained. “By our estimates, 95% of fertile land is in physically in the hands of only 2% of the population, and it is not difficult to see who comprises that 2%.”


Not that Afghanistan’s landholding ratios by themselves are alarming by third-world standards. South Africa, to cite just one example, has failed miserably in delivering land reforms, a good 13 years since the end of Apartheid. What makes Afghanistan unique is that the impact of those ratios is compounded by the absence of a secure land title system, and by the desire of those influencing the use of agricultural land to grow poppies for profit.


But wait. There are a few more depressing statistics which you need to be aware of. The wholesale price of a gram of heroin is a mere US$2.50 in Afghanistan. Just beyond Afghanistan’s borders, in Iran and in Pakistan, a gram changes hands at US$3.50. That figure reaches US$10 by the time the heroin reaches Turkey, Kosovo and Albania. Today’s wholesale price in Europe ranges between US$22 and US$33 per gram. One final note: retail prices can be more than 10 times higher on the street.


Just do the maths, and figure out the phenomenal impact Afghan poppies are having on an entire array of economies. At the same time, the situation inside Afghanistan, and along the Afghanistan-Pakistan border, can only be subject to a credible determination once some basic questions are answered.


Are those so-called religious extremists—Al Qaeda and the Taliban—simply glorified drug traders? “They don’t seem to be doing much praying in the mosques around here,” an Afghan border guard told a British journalist at a busy Kandahar border crossing, as donkey-drawn carts moved weapons, whiskey and pirated Bollywood videos in the vicinity.


The second set of questions must be addressed with an elevated degree of seriousness. Who else, besides these militants, is participating in the opium wealth? Are law enforcement agencies on both sides of the border actively or passive players? Finally, what kind of war is NATO actually fighting when more than a few backers of the Karzi government are widely suspected of being direct beneficiaries of the Afghanistan’s opium export boom?


Without comprehensive answers to a series of fundamental and disturbing questions, talk of Afghanistan’s reconstruction is meaningless, without context, without any grasp of the reality on the ground.
